Registration Form

PPTRC’s Spring 2010 Registration

April 5 - May 29 (8 weeks)

Registrations Due March 12, 2010 by 5pm

You may register for classes in one of two ways.

  1. Fill out the Online Registration form.

  2. Print out the .pdf form, fill it out and fax or mail it in.

REGISTER NOWRegistration is on a first-come, first-serve basis based on the number of riders appropriate for each class and how best to keep the classes similar in abilities.  PLEASE return this ASAP for a better chance at your 1st choice.

Instructions:

  • Please read the Cover Letter.

  • Review the session schedule, and select a first and second choice class that match the class grouping you have been assigned. 

    • If you have questions about class groupings, talk with the Program Manager.

    • If you do not see a class that fits your schedule, please call the office and we will do our best to accommodate you.

    • If there is room in the class you will be registered for your first choice.  If not, you will be registered for your second choice or we will call to try to find another time that is available.

  • Please fill in all contact information. If you are a returning rider, this helps us verify that the information we have on file is correct.

  • If NOT registering online, return this registration by:  mail, fax, PPTRC drop box (outside office door), or in person.

  • Payment must be received before reservations are finalized.  You may phone the office with your credit card information, mail in a check or money order, or pay cash in-person.  You will receive a confirmation by email or phone.

Other information:

  • Please Read the Program Profile document.  We have new policies/procedures that need to be completed BEFORE registering!

  • New Rider Information:

    • New riders cannot register for classes until an evaluation by PPTRC has been completed.  Riders transferring from hippotherapy to therapeutic riding also require an evaluation by PPTRC prior to registration.  To schedule your new rider evaluation interview with Program Manager Nancy Marshall at the center (call 719-495-3908).

    • If you are new OR if we do not have current (within one year) forms, print and fill out the  Rider Release Form and Physician Form (or the rider will not be allowed to ride).

    • New Riders - must print and fill out the Rider Health History and bring to the rider evaluation. 

    • New Riders - we request (not mandatory) that you have any therapist you work with complete the Therapist Summary.

  • Types of classes:

    • Group classes consist of 3-5 riders and are one hour long with approximately 45 minutes of lesson time.

    • Semi-private lessons are two riders and are 45 minutes long, with approximately 35-40 minutes of lesson time.

    • Private lessons are one rider and are 45 minutes long with approximately 40 minutes of lesson time (private lessons are scheduled by request only).

    • PPTRC reserves the right to conduct ground lessons if we do not have the proper number of volunteers needed to safely conduct a riding class.

  • In the event a class does not fill, PPTRC reserves the right to cancel that class. PPTRC reserves the right to cancel classes that do not fill and to change the class time to accommodate the majority of riders.  Please understand that PPTRC needs to schedule instructors, volunteers, and activities, so we are counting on you to get your registration papers and payment in on time.

  •  Rider Support Fund (RSF)If you are in need of assistance:

    • Please read the RSF Cover Letter.

    • Fill out the online Rider Support Fund and submit it.

    • Or print out the RSF.pdf file and return it with your registration.

    Remember, applications for RSF must be submitted with the registration.

 

Please be aware of the following:

  • Appropriate dress - long pants and sturdy, closed-toe shoes are required.  Dress in layers - the weather here can be different than in town, and remember sunscreen and water for warm days and jackets and hats for cooler days.

  • If you are interested in arriving early to get your own horse ready, please let the instructor know so that they can plan to have a volunteer ready to assist.

  • If you are unsure as to the status of your class due to weather reasons, call the center at 495-3908 and listen to the voice mail greeting.  If there is no message that classes have been cancelled, the class is presumed to be on schedule.

  • PPTRC Instructors and staff hold the right to cancel classes at any time for safety reasons.
